About the Job:
This is a community-based role that requires frequent travel to various locations throughout the county, including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
To be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.
To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs
You will need to have knowledge of the legislation around mental health and The Mental Health Act. You will need strong communication skills and a commitment to teamwork and inter-agency collaboration. We would like to work with an individual who wants to utilise their skills to effect positive changes for our service users.
We need a person who can demonstrate passion and commitment to mental health issues, and have an approach to excellent client-centred care along with working flexibly as part of a team, to deliver innovative health and social care. You will receive regular support and supervision from the Management and Leadership team.
