About the Job:
- Support the running of Depot, Clozapine, and Wellbeing Clinics, delivering safe and effective clinical care.
- Undertake venepuncture, physical health monitoring, and ordering medication as required.
- Work closely with pharmacists, registered nurses, and wider MDT colleagues.
- Provide high quality, evidence-based nursing care within your scope of practice.
- Monitor and respond to changes in a patient’s mental and physical health, escalating concerns appropriately.
- Maintain accurate and up-to-date patient records in line with trust policies and NMC standards.
- Help maintain a safe, clean and well-organized clinic environment.
- Support patients to improve and maintain their mental, physical and emotional wellbeing.
- Work collaboratively with colleagues to support safe discharge planning and continuity of care.
- Contribute to audits, quality improvement and clinical governance activities.
