About the Job:
- To perform peripheral cannulation and venepuncture in accordance with established protocols.
- To perform port-a-cath procedures according to established protocols.
- To act as an assessor to district nurses in the assessment of the administration of intravenous antibiotics.
- To provide clinical support and advice to the patient and district nurses in the absence of the IV team leader, e.g., during holidays or study leave.
- To work alongside the district nurse teams to support them in providing intravenous therapy within the patients’ home environment.
- To work under the direction of the IV Team Leader.
- To work within Trust policies and guidelines and follow the intravenous therapy patient care pathways.
- To provide a safe, seamless transition of care by using a multidisciplinary approach to care.
- To work without direct supervision of the IV team leader in the patient’s own environment and also within a clinic setting. Reporting to the IV Team Leader.
- To support the assessment, development, implementation, and evaluation of care.
- To provide safe discharge of patient care from the pathway.
- To provide accurate written communication of the patient’s progress to the IV team leader.
- To support the training/skills/competencies of other community nurses within intravenous antibiotic therapy.
- Effectively communicate verbally with patients, their families, and other health care professionals in the course of their work.
