About the Job:
Responsibility for Patients, Families, and Carers
- Provide direct nursing care to palliative and end of life patients, including catheter care, pressure care, simple dressings, and support with daily living.
- Implement individualised care plans, monitor changes in patient condition, and escalate concerns to GPs or out of hours services when required.
- Accurately document all care delivered during and after each visit.
- Offer emotional, psychological, and practical support to families and carers, ensuring they feel informed and supported throughout the care process.
- Provide information on palliative care services, statutory and voluntary support, and Hospice at Home when requested.
- Instruct carers on the safe use of equipment such as hoists following professional assessment, ensuring correct use and reporting any faults appropriately.
Administration and Self Development
- Support induction of new staff through shadowing opportunities.
- Maintain compliance with all mandatory training requirements.
- Plan and organise home visits effectively.
- Participate in supportive conversations throughout the year.
- Attend monthly team meetings and complete annual appraisals.
