About the Job:
- As a UCR Nurse you will provide high-quality, patient-centred nursing care to adults in their own homes, undertaking holistic assessments, physical examinations and clinical interventions.
- You will respond to urgent and unplanned referrals, prioritising visits according to clinical need and supporting delivery of the UCR service.
- Responsibilities include medicines management, complex wound care, catheter management, palliative and end of life care, and maintaining accurate electronic patient records using EMIS.
- You will work closely with GPs, community services and acute services to coordinate care, promote independence and avoid hospital admission.
- The role also includes supporting service improvement, clinical audit, supervision of junior staff and students, and acting as a clinical role model within the community nursing team.
Shifts cover the service hours of Monday to Sunday between 8am and 8pm. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
