About the Job:
- Act at all times with compassion, be committed to delivering high-quality care, use effective communication, be competent, and have courage when needed.
- Monitor the quality-of-care planning, implementation, and evaluation.
- Inform the Community Matron (Band 7) of any concerns with patients, relatives, visitors, or staff that may compromise patient care.
- Communicate effectively with colleagues, patients, and carers so that information is shared in order to meet patients’ needs
- Act as a role model in the promotion of person-centered practice, and challenge practice that is not person-centered.
- Ensure that discharge and transfer planning for patients is done in a proactive and interdisciplinary manner
- Assess and maintain the skills and competencies of the team
- Support students in their learning and development
- Ensure new staff receive a proper induction so that they understand acceptable practice and standards
- Develop own professional skills and ensure these are shared with the team and wider division
- Provide management responsibility for the team in partnership with the Community Team Matron. This may include annual leave management, sickness/absence management, performance management, training and development, appraisals, and incident and complaints management.
- Depute for the Integrated Community Team Matron as required
- Building and maintaining effective relationships with all stakeholders
- Ensure effective management of own and team caseload
