About the Job:
- Responsibility for arranging and providing physical health checks for patients with a serious mental illness.
- Reviewing and evaluating physical health and wellbeing care plans, identifying, organising, and signposting to other resources as appropriate.
- Providing assessments/interventions/activities within your scope of practice and level of responsibility.
- Completing relevant paper and electronic documentation in line with the associate practitioner role and local processes, in a timely and effective manner.
- Provide information, advice, and signposting to promote good physical health and well-being, e.g., smoking cessation, healthy diet, exercise, alcohol consumption, employment support, etc.
- Encouraging patients to access suitable follow-up monitoring and interventions for any physical health concerns.
- Escalating any physical health concerns to appropriate clinical staff, including GPs.
- Supporting the delivery of a group health and exercise program for these patients alongside other members of the team.
- Working flexibly to provide the service delivery need across Herefordshire and Worcestershire.
- Working with external stakeholders, signposting to supportive services, particularly in the VCSE sector.
