About the Job:
- Ensure that self and other professionals adhere to infection control best and planning and delivering care to patients.
- Maintain accurate and clear documentation of all nursing care and communication with the multidisciplinary team.
- Communicate with patients and relatives in a sensitive manner, providing information regarding care and progress as required.
- Liaise with the multidisciplinary team to ensure clinical care is optimised from admission to discharge.
- Facilitate discharge of patients to enable safe, appropriate and timely discharge with suitable ongoing arrangements for care needs.
- Communicate effectively with the matron, bed management team and discharge coordinators to optimise use of inpatient beds.
