About the Job:
Main duties of the job:
- Assessment of patient care needs, implementation of individualized care planning, and then evaluation of planned care.
- Updating patient-specific risk assessments using the trust’s electronic recording systems, ensuring accurate, contemporaneous documentation standards are followed.
