About the Job:
What you’ll do:
- Assess patients’ needs and deliver safe, person-centered nursing care in line with care plans.
- Triage, assess, and provide unplanned care to support patients in the community and help prevent hospital admissions
- Work with multidisciplinary teams and partner organizations to coordinate effective care and referrals
- Maintain accurate clinical records and contribute to high standards of patient safety and care quality
- Support learning and development by sharing knowledge with colleagues, students, and carers
Essential requirements:
- Registered Nurse with current NMC registration
- Excellent clinical assessment and communication skills
- Commitment to delivering patient-centered care
- Ability to work independently and as part of a multidisciplinary team
- Full UK driving licence and access to a vehicle suitable for the role
