About the Job:
Key Responsibilities:
- Adhere to the code of practice/conduct of your registering body at all times
- Discharge professional responsibilities under the Mental Health Act (1983) and the revised subsequent act, the Mental Capacity Act (2005), and the NMC Professional Code of Conduct
- Coordinate and monitor the delivery of person-centered healthcare to a designated group of people who use our services, using complex clinical decision-making where appropriate.
- Develop, implement, and evaluate evidence-based programs of care; set standards of care; and liaise with other agencies, particularly in relation to people discharged under the Care Programme Approach.
- Participate in the multidisciplinary assessment of individuals with mental health problems or vulnerabilities living in the community by providing a community mental health perspective and expertise
- Ensure that a risk assessment is completed for service users, especially if there is a history of or a potential for risk.
- Be aware of the potential for vulnerability to abuse amongst the people who use this service, and use the local policy to inform appropriate personnel in line with safeguarding vulnerable adults and children and SABP policies
- Ensure that a carer’s assessment is offered to carers of people who use services, in line with Standard 6 of the National Service Framework.
- Help develop and maintain effective joint working arrangements with colleagues in the community mental health services, residential services, primary health care, social services, and voluntary services, thus facilitating the co-ordination and comprehensiveness of provision.
- Utilize the full range of mental health skills to participate in the assessment, care, and support of people who use this service. Use specialized knowledge, experience, and skills in direct clinical work and act as a resource for the team and other agencies.
- This may include work in rehabilitation, social interventions, development of social networks and accessing other services/resources, mental health promotion, counselling and intercultural therapy, and encouraging service user participation at all times.
- Participate in the provision of community mental health advice and consultation to colleagues, community groups, and organizations.
- Promote the recovery model to the people who use the service and their carers
- Act in a manner to respect the customs, individuality, values, sexuality, and spiritual beliefs, actively supporting the individual to fulfill these.
- Ensure detailed and appropriate records are maintained to demonstrate that contact with people who use services and their carers is meaningful and effective.
- Utilize a range of specialist skills appropriate to the needs of individual patients and the clinical setting in which they are required to be seen and work autonomously
- Responsible for the full assessment of the psychiatric/psychosocial needs of those patients who have been referred to the service, including the comprehensive assessment of risks to self and others
- Responsible for the development, implementation, and evaluation of robust discharge plans based on a clear assessment and formulation of the person’s needs, which may include urgent referral to secondary mental health services and social and voluntary services
