About the Job:
The role of the Nursing Associate is to work with the Registered Community Nurses and other members of the multiprofessional team, providing and monitoring care and contributing to the ongoing assessment,as agreed with supervising clinician(s) and in line with the service pathways. They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi professional team.
This role will support the Community Nursing team with organising the patients on the end-of-life or Continuing Care at Home caseload. Support the team with coordinating and delegating work to the Health Care Assistants. Support the team with organising other resources such as equipment. Support the team by providing supervision, teaching and coaching. To review patients on the case load and adjust care plans.
