About the Job:
The postholder will carry responsibility for the triage, assessment and review of patients who require interventions within primary care or onward referral to appropriate services.
As a single point of access service, the workload is rich and diverse, including assessment of mild/moderate mental health difficulties, linking in with our Medical Provision, and referring on to Specialist Services, including working closely with Talking Therapies colleagues, as well as assessment of complex and severe mental health problems and referring on to appropriate services, depending on needs and complexities. We also have Medical Provision within the service consisting of Psychiatrists and an advanced clinical practitioner.
You will work as part of an experienced MDT, including mental health nurses, psychiatrists, psychologists, and social prescribers, and liaise with other teams.
