About the Job:
Job responsibilities:
- To lead the assessment and re-assessment of patients and carers receiving home nursing and volunteer support.
- To coordinate and manage the Hospice at Home Day/Night Nursing Team, acting as supervisor and coordinator for the Day/Night Nursing Service in the absence of the Head of Clinical Services.
- Assist the Head of Clinical Services to develop and support nurses and the wider team through an effective appraisal and development review process, identifying learning needs and providing training opportunities.
- Participate in direct provision of patient care and liaise with members of the multi-disciplinary team and palliative care team in order to achieve this.
