Qualifications
What we’re looking for:
- NVQ Level 2 in Health and Social Care or equivalent Level 2 qualification (e.g., NCFE, BTEC), plus functional skills in numeracy and literacy at Level 2
- At least 6 months of recent care delivery experience in a health or social care setting
- Strong communication skills with the ability to hold person-centered, empathetic conversations and work effectively within a multidisciplinary team
- Knowledge of palliative and end-of-life care, including holistic care planning and symptom management
- Ability to plan and prioritize care, respond to changes in patient condition, and escalate appropriately
- Full driving license and access to a vehicle
