About the Job:
What we are looking for:
- Proven experience in providing care in a community context within Health or Social Care settings, along with an understanding of end-of-life care for patients.
- NVQ Level 2 in Health and Social Care or equivalent (Preferred)
- Be person-centred and compassionate when caring for the person and those important to them.
- Ability to develop and evaluate care management plans that meet the complex needs of the person, and those important to them.
- Emotional resilience and the capacity to work independently.
- A valid driving license and access to a car for travel to patient homes are required.
