About the Job:
- Deliver high-quality, person-centered care to older adults with functional mental health needs.
- Support patients with activities of daily living, ensuring dignity, respect, and independence are maintained
- Observe, monitor, and accurately record patients’ mental state, behaviour, and physical health, reporting concerns promptly to registered staff
- Undertake enhanced therapeutic observation of care in line with risk assessments and care plans
- Assist with physical health monitoring, including vital signs, nutrition, hydration, mobility, and skin integrity
- Support patients to engage in therapeutic, social, and recreational activities
- Contribute to the maintenance of a safe, calm, and therapeutic ward environment
- Respond appropriately to distress, confusion, and behaviours that challenge, using de-escalation techniques
- Communicate effectively with patients, carers, and members of the multidisciplinary team
- Maintain accurate and timely documentation in line with Trust policy
- Support and act as a role model for students and new starters as appropriate
- Adhere to infection prevention, safeguarding, health and safety, and confidentiality requirements
- Uphold Trust values and contribute to service improvement and quality of care
