About the Job:
- The postholder will hold responsibility for a defined caseload of patients drawn from a GP practice population.
- He/she will hold a specialist practice qualification and be skilled in assessment of patient care needs and in the development, implementation and evaluation of care packages and acquire and employ active nurse prescribing skills.
- He/she will work without supervision, and will, additionally, assist the Operational Lead by conducting delegated tasks instrumental in team management and care co-ordination.
- The postholder will assume responsibility for directing team co-ordination.
- The postholder will lead in organising the induction and clinical supervision of students and new team members.
- The postholder will accept the responsibility for doing a quality job, aspire to achieve, share a corporate pride in achievements and strive to act as an ambassador for good quality
