About the Job:
- Join our Community Nursing Team delivering high-quality care to housebound patients in their own homes. As a Band 5 Community Staff Nurse, you will manage a defined caseload, using your clinical knowledge and skills to provide safe, effective, and patient-centred care.
- You will undertake holistic assessments of routine and complex patients, planning and delivering care in partnership with patients and their families.
- Working largely independently, you will carry out daily visits, document care accurately, and escalate concerns appropriately.
- You’ll participate in team handovers, liaise with GPs and multidisciplinary colleagues, and support tasks such as equipment ordering.
- A key part of the role is promoting independence, offering health education, and supporting patients to self-manage.
- You will work collaboratively across health and social care to ensure patient safety and wellbeing.
- You’ll also support the development of students and junior staff, acting as a role model and supervisor.
- Opportunities exist to take on additional responsibilities, champion roles, and deputise for senior staff by coordinating visits and leading handovers.

Additional Working Pattern: 7.5hrs a day Monday to Friday with rostered late and weekends on a rotational basis
