About the Job:
- Undertaking urgent assessments of mental health in various settings in the community to include risk assessment
- Triaging paper referrals to include telephone triage to determine the most appropriate service for ongoing mental health support
- Accessing various data bases to seek information to include Care Notes, ICE and Adult Social Care records. Recording on our Care Notes data base
- Professional telephone conversations with partner agencies, patients, relatives, carers
- Arranging admissions to hospital where any assessment you have undertaken requires this, this can include liaison with the Approved Mental Health Professional Service for anyone requiring admission under the Mental Health Act.
- Liaison with the Community Area Consultant Psychiatrists as required
- Liaison and referral, to include joint visits where indicated, with our Hospital at Home Team
- Attending professionals meetings
- The role does not involve holding patient cases for any prolonged intervention.
