Main duties of the job:
- To assist in assessing people’s health and social-related needs. This will include:
- Assisting in the assessment of people’s health and social care needs
- Taking and interpreting physical measurements and samples from named patients as delegated by the team
- Monitoring changes in patients, correctly interpreting their meaning, and informing the team when changes might be significant
- To assist in planning, delivering, and evaluating nursing care and rehabilitation programs to address people’s health and social needs according to their care plans.
