About the Job:
The Advanced Nurse Practitioner will work as part of the practice team to provide safe, effective, and high-quality care to patients.
Main duties will include:
- Assessing, diagnosing, and treating patients with a range of acute and long-term conditions.
- Carrying out clinical assessments and making appropriate treatment decisions.
- Prescribing and managing medicines within their scope of practice.
- Supporting patients with ongoing health needs and care plans.
- Providing health advice, preventative care, and health promotion.
- Working closely with GPs, nurses, and the wider healthcare team to provide joined-up patient care.
- Making referrals where appropriate and following clinical pathways.
- Keeping accurate patient records and ensuring clinical documentation is completed.
- Taking part in audits, service improvements, and practice meetings.
- Supporting the learning and development of colleagues where appropriate.
- Keeping up to date with clinical guidance, training, and professional requirements.
The postholder will play an important role in improving patient access, supporting the clinical team, and delivering excellent care to our patients.
